Keystone Ridge Post Acute Nursing & Rehabilitation is seeking a dedicated MDS Coordinator to join their team. This role involves collaborating within an interdisciplinary team to develop individualized care plans, monitoring documentation for accuracy and compliance, supporting quality improvement initiatives, and maximizing reimbursement while promoting high-quality resident care. The ideal candidate will possess strong knowledge of long-term care regulations, assessment processes, and excellent organizational and communication skills.
