About The Position

We are seeking a detail-oriented and compassionate RN Nurse Assessment Coordinator (NAC) to join our team. In this role, you will be responsible for completing accurate and timely resident assessments, ensuring regulatory compliance, and supporting high-quality, person-centered care. The NAC works closely with interdisciplinary team members to assess resident needs, develop individualized care plans, and promote positive clinical outcomes. After 90 days, this role will be added to the on-call rotation. Wage Scale Based on YOUR years of experience.

Requirements

  • Current RN license South Carolina
  • As a condition of employment, nurses must meet Primary State of Residence (PSOR) and Nurse Licensure Compact (NLC) requirements.
  • Nurses must maintain an active nursing license in their Primary State of Residence.
  • Candidates residing outside of South Carolina must maintain appropriate licensure in accordance with their Primary State of Residence and NLC requirements.
  • Strong assessment, documentation, and teamwork skills

Nice To Haves

  • Long-term care or MDS experience preferred
  • Nurses whose Primary State of Residence is South Carolina must hold a South Carolina nursing license and a South Carolina driver's license or state-issued ID with a South Carolina address.

Responsibilities

  • Complete and submit Minimum Data Set (MDS) assessments and all related documentation accurately and within required timeframes.
  • Ensure compliance with federal, state, and facility regulations related to resident assessments and reimbursement processes.
  • Monitor assessment schedules and maintain accurate clinical documentation to support quality care and regulatory standards.
  • Provide guidance and support to nursing staff regarding assessment practices, documentation requirements, and care planning.
  • Conduct nursing assessments and collaborate on care plans
  • Support nursing staff and participate in QA reviews and meetings
  • Collaborate with physicians, therapists, social services, dietary staff, and other team members to ensure coordinated resident care.
  • Stay current on MDS guidelines, regulatory changes, and best practices in long-term care.
