About The Position

Under general supervision of the Director of Nursing and in accordance with the policies, procedures and guidelines established within Bronson Commons, the MDS (Minimum Data Set Assessment) nurse completes the MDS assessments on patients, completes insurance certifications and re-certifications and prepares responses for insurance medical record audits. Position plays a critical role in ensuring the accuracy of MDS assessments to ensure the optimization of reimbursement, compliance with regulations and the accuracy of data reported that determines facility 5 Star rating including CMS Quality measures.

Requirements

  • Associates degree, Diploma, or Bachelor of Science from an Accredited Nursing Program prior to employment, and successful completion of clinical orientation program
  • Licensed Registered Nurse in good standing with the State of Michigan
  • MDS certification required within one year of hire.
  • BLS certification required by completion of core orientation.
  • Must have excellent communication, interpersonal and organizational skills.
  • Must be actions willing to accept a high-level of responsibility and demonstrate accountability for.
  • Must be able to prioritize using critical thinking skills.
  • Must be willing to seek additional information from a variety of sources.
  • Ability to utilize word processing, spreadsheet, keyboard skills, presentation programs, and other software relevant to the job.
  • Must be highly organized and have the ability to work efficiently with a high level of accuracy.
  • Work which produces levels of mental/visual fatigue which are typical of jobs that perform a wide variety of duties with frequent and significant uncontrollable deadlines. Work may include the operation of and full attention to a personal computer or CRT up to 40 percent of the time.
  • Job definitely requires above average strength and/or stamina to regularly lift weights up to 100 pounds and occasionally assisting with heavier tasks or expending the equivalent effort in pushing, pulling, or otherwise handling material, equipment and other objects. Work may be performed in positions requiring considerable expenditure of effort in reaching, climbing, maintaining balance, or manipulating heavy objects.

Responsibilities

  • Lead role in managing/determining MDS process including: a. Determination of the most optimal ARD Assessment Dates. b. Ensuring timely completion c. Monitoring and auditing for MDS accuracy
  • Completes Minimum Data Set Assessments on short stay patients and residents.
  • Tracks Priority Health Dashboard Data and Outcomes
  • Processes MPRO and KePRO audits and appeals
  • Completes chart review and completes appeal submittals for all insurance audits
  • Completes insurance certifications and re-certifications
  • Issues NOMNC and ABN forms to patients
  • Attend Care Conference Meetings
  • Attend Discharge Planning Meetings
  • Participates in nursing stand-by coverage and may be called upon to work as a charge nurse.
  • Other duties as assigned
  • Employees providing direct patient care must demonstrate competencies specific to the population served.
  • The MDS Nurse collects comprehensive data pertinent to the residents’ health or the situation in order to accurately complete assessments.
  • Assesses the resident’s physical and psychosocial needs
  • Analyzes assessment data to identify appropriate resident needs
  • Documents nursing history
  • Reviews vital signs to assess condition of resident and detect deviations from normal.
  • The MDS Nurse analyzes the assessment data to determine patient problems or issues
  • Uses the assessment data to identify resident problems
  • Confirms problems with resident and family when appropriate
  • Documents the expected outcomes and plan of care related to the problems in the medical record
  • The MDS nurse identifies expected outcomes for a plan individualized to the resident
  • Utilizes resident/family input when defining outcomes. Incorporates the resident values, ethical and cultural considerations.
  • Coordinates discharge planning for outcomes not achieved utilizing the continuum of care available.
  • The MDS nurse develops a plan that prescribes interventions to attain expected outcomes.
  • Incorporates Clinical Practice Guidelines into the plan when appropriate
  • Develops individualized interventions with resident input when appropriate
  • Considers data from other members of the interdisciplinary team when developing the plan of care
  • The MDS nurse implements the identified plan.
  • Implements appropriate interventions.
  • Administers prescribed medication and treatment in accordance with approved nursing techniques and polices
  • Utilizes evidence-based interventions specific to the residents plan
  • Maintains awareness of comfort and safety needs of the resident.
  • Initiates a resident education plan according to the individualized needs of the resident, and coordinates the implementation of the plan.
  • Manages the care plan for residents by organizing and delegation resident care interventions to be implemented. Evaluates the effectiveness of care given by self and others.
  • The MDS nurse evaluates progress toward attainment of the expected outcomes
  • Evaluates the effectiveness of the planned interventions and the resident response toward achievement of the expected outcomes.
  • Documents the resident response toward the expected outcomes.
