About The Position

St. John’s is seeking a dedicated and talented MDS Registered Nurse Lead Coordinator to join their family. This role is at the forefront of innovative senior services delivery in the community. This is a full-time, 40 hours per weekday position located at St. John’s Home.

Requirements

  • Current New York State Registered Professional Nursing License required.
  • Current CPR certification.
  • PRI certification and AANAC MDS 3.0 certification within 18 months of hire.
  • Three years clinical nursing experience, preferably in a long-term care setting.
  • Working knowledge of MDS and Medicare A reimbursement.
  • Must possess good organizational, leadership, and communication skills.
  • Ability to interact with Elders and care for their needs.
  • Technical expertise in MS Word, email, AOD and charting.
  • Working knowledge in MS Word, email, AOD and EMR charting.

Responsibilities

  • Completes the Minimum Data Sets (MDS) as assigned, while remaining open to learning and mastering other aspects of Reimbursement.
  • Assisted with oversight and support of LPN Assessment Coordinators (LPNACs).
  • Coordinated and scheduled Minimum Data Set (MDS) assessments.
  • Audited MDS assessments to ensure quality measures, regulatory compliance, completeness, and accuracy.
  • Conducted audits for Patient-Driven Payment Model (PDPM) and New York State Case Mix Index (CMI) accuracy.
  • Assisted with MDS completion as needed to ensure timely submission.
  • Performed final ("check") reviews of completed MDS assessments for accuracy and compliance.
  • Provided education and guidance to LPNACs regarding the RAI Manual and proper MDS completion.
  • Served as a liaison with physicians and other medical providers to obtain documentation and clarify clinical information as needed.
  • Provide the interface for communication of resident care needs with members of other disciplines, utilizing an in-depth knowledge base to ensure comprehensive follow-through on problems.
  • Compare and interpret previous and present data when problem solving and report pertinent data in records.
  • Recognize and report safety hazards.
  • Utilize knowledge base of group dynamics when working with residents, participating in staff and interdisciplinary meetings.
