About The Position

Do you thrive in a role where attention to detail and teamwork come together to support quality care through the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) process? Frasier is hiring for a full time MDS / RAI Coordinator. This role involves leading and coordinating the RAI process, completing and submitting MDS assessments accurately and on time, and collaborating with various team members to ensure compliance with regulatory requirements. The coordinator will also monitor assessment schedules, support care planning, and serve as a resource for MDS and RAI requirements, including participation in an on-call rotation.

Requirements

  • Current, active RN license in Colorado (in good standing)
  • At least 3 years of experience as a Registered Nurse, including 1+ year with MDS completion
  • Solid understanding of Medicare, Medicaid, and state/federal regulations related to MDS
  • Working knowledge of ICD-10 coding
  • Experience with electronic health records (PCC experience a plus)
  • Strong computer skills and comfort with clinical documentation systems
  • Clear communication skills and the ability to work effectively with residents, families, and team members

Responsibilities

  • Lead and coordinate the Resident Assessment Instrument (RAI) process across disciplines
  • Complete and submit Minimum Data Set (MDS) assessments accurately and on time
  • Collaborate with nursing, therapy, and other team members to gather and validate information
  • Ensure compliance with CMS and state regulatory requirements
  • Monitor assessment schedules and follow up to keep everything on track
  • Support care planning by ensuring assessments reflect each resident’s needs
  • Serve as a resource to the team on MDS and RAI requirements
  • Participate in on-call rotation responsibilities
