About The Position

EHM Senior Solutions is excited to partner with a new MDS Nurse. The primary purpose of your job is to conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations and guidelines that govern the resident assessment, including the implementation for RAP's and Triggers.

Requirements

  • Associate's or Bachelor's degree in Nursing from an accredited institution.
  • Current Registered Nurse (RN) license in the State of Michigan
  • Minimum of 2 years of clinical nursing experience, preferably in a long-term care or rehabilitation setting.
  • Experience with the Minimum Data Set (MDS) assessment process and care planning in a skilled nursing facility.
  • Proven track record of working collaboratively with interdisciplinary teams to enhance resident care.
  • Strong understanding of MDS 3.0 and its application in resident assessments and care planning.
  • Excellent organizational skills with the ability to manage multiple priorities and meet deadlines.
  • Proficiency in electronic health record systems and MDS software applications.
  • Effective communication skills, both verbal and written, with the ability to interact professionally with residents, families, and staff.
  • Detail-oriented with a commitment to accuracy and compliance in documentation and reporting.
  • Ability to educate and train staff on MDS processes and regulations.
  • Compassionate and empathetic approach to resident care, with a focus on enhancing quality of life.

Responsibilities

  • Conduct comprehensive resident assessments using the Minimum Data Set (MDS) per CMS guidelines.
  • Collect, review, and validate clinical data from multiple sources — nursing notes, therapy, dietary, and physician documentation.
  • Develop and update individualized person-centered care plans based on MDS findings and resident needs while ensuring compliance with regulatory standards.
  • Collaborate with nursing, therapy, social services, and dietary staff to implement care plans
  • Maintain knowledge of current MDS criteria, ensuring assessments align with billing and reimbursement requirements.
  • Monitor MDS transmission deadlines and ensure timely submission to CMS.
  • Assist in preparing for state surveys and audits by maintaining accurate, compliant resident documentation.
  • Monitor Quality Measures and provide feedback to the clinical team to drive continuous improvement.
  • Educate nursing and interdisciplinary staff on MDS processes, documentation standards, and coding accuracy.
  • Serve as a clinical resource for staff regarding assessment documentation, diagnosis coding, and CMS updates.
  • Accurately code conditions, treatments, and services based on chart review and clinical guidelines.
