About The Position
Cedar River Healthcare Center is hiring a full-time MDS Nurse (LPN/LVN) to support resident care planning and accurate, timely documentation. The role works closely with an interdisciplinary team and contributes to resident care outcomes, reimbursement accuracy, and quality measures.
Requirements
- Current, active LPN license in the state of Washington.
- Strong organization and attention to detail.
- Strong communication and teamwork skills.
Nice To Haves
- MDS experience in a skilled nursing setting is strongly preferred.
- Knowledge of PDPM, the RAI process, and CMS regulations.
Responsibilities
- Complete accurate, timely MDS assessments according to RAI and CMS guidelines.
- Coordinate and lead care plan meetings with the interdisciplinary team.
- Review clinical documentation to ensure it supports MDS coding.
- Monitor assessment schedules and submission deadlines.
- Collaborate with nursing, therapy, social services, and dietary teams.
- Help support accurate reimbursement and quality measures.
