About The Position
Generations at Applewood in Matteson, IL, has an immediate opening for a MDS Nurse (RN or LPN). This is a full-time position. The salary range is $90k-$95k for an RN, depending on MDS Certification and experience, and $79k-$85k for an LPN, also depending on MDS Certification and experience. We offer a full benefit package and a collaborative, supportive work environment. This role is ideal for a dedicated, experienced MDS nurse and involves being part of a two-person MDS team. Generations at Applewood is a 24-hour rehabilitation and skilled nursing community offering quality healthcare services to senior residents throughout the Chicagoland area.
Requirements
- Current RN or LPN licensed from the State of Illinois
- Knowledge of Minimum Data Set (MDS), ICD-10 Coding
- Two (2) to three (3) years of applicable experience as a MDS Nurse
- Must be CPR Certified
- Experience in a long term care setting
Nice To Haves
- American Association of Nurse Assessment Coordination (AANAC) certification preferred
- RAC-T Certified preferred
Responsibilities
- Collaborating with the Interdisciplinary Team
- Plan, organize, develop and implement the MDS assessments and care plans
- Update records and assessments, ensuring information is consistent with patient care plans and that scoring is accurate
- Accountable for accurate MDS scoring
