About The Position

St. Francis Healthcare is seeking a knowledgeable and detail-oriented MDS Nurse (LVN/LPN) to join our dedicated clinical team. This role is essential in ensuring accurate resident assessments, regulatory compliance, and quality care planning within our skilled nursing environment. The MDS Nurse (LVN/LPN) is responsible for coordinating and completing Minimum Data Set (MDS) assessments in accordance with federal and state regulations. Requirements

  • Current and active LVN/LPN license in the state of practice required
  • Knowledge of MDS 3.0, RAI process, and Medicare/Medicaid reimbursement guidelines
  • Strong organizational skills and attention to detail

Nice To Haves

  • Prior experience in a skilled nursing facility or long-term care setting preferred
  • Experience with electronic health records (EHR) systems preferred

Responsibilities

  • Complete MDS assessments and Care Area Assessments (CAAs) within required timeframes
  • Ensure accurate and timely resident documentation to support care planning and reimbursement
  • Maintain compliance with CMS, Medicare, Medicaid, state, and facility guidelines
  • Monitor resident progress and update care plans as appropriate
  • Collaborate with interdisciplinary teams to develop individualized resident care plans
  • Participate in resident care conferences and quality improvement initiatives
  • Work closely with nursing, therapy, dietary, and social services teams to gather and review resident information
  • Communicate effectively with residents and families regarding care planning and services
  • Ensure MDS submissions meet all regulatory and reimbursement requirements
  • Assist during audits and surveys by providing accurate documentation and reports
  • Stay current on changes related to MDS 3.0, RAI processes, and reimbursement regulations
  • Educate and support staff on proper documentation standards and MDS requirements
