About The Position
Linley Park Post Acute is an 88-bed skilled nursing and rehabilitation facility located in the heart of Anderson, South Carolina. We are proud to deliver high-quality, patient-centered care in a collaborative, team-focused environment. We are seeking a detail-oriented MDS Nurse (LPN) to join our team. This role is critical in ensuring accurate clinical documentation, regulatory compliance, and optimal reimbursement—while supporting exceptional outcomes for our residents.
Requirements
- Active, unencumbered LPN license in South Carolina
- Experience in a skilled nursing or long-term care setting
- Strong knowledge of MDS 3.0, RAI process, and reimbursement guidelines
- Experience with EHR systems (PointClickCare preferred)
- Proficiency in Microsoft Office (Excel, Word, Outlook)
- Strong attention to detail, organization, and analytical skills
- Ability to collaborate with interdisciplinary teams
