About The Position

The MDS Nurse is responsible for completing the Minimum Data Set (MDS) for each resident in compliance with state and federal regulations to accurately reflect the resident's status. This role involves working with the interdisciplinary team to develop and update care plans, communicating with residents and families, and staying informed about resident condition changes.

Requirements

Must be an L.P.N. or R.N. currently licensed and in good standing with the Board of Nursing in the state in which the facility is located.
Must be able to tolerate a high pace as typical for a nursing facility.
Must be able to take and give instructions well.
Must enjoy working with the elderly.
Must be able to tolerate prolonged standing and walking.
Must be able to sit for prolonged periods of time.

Responsibilities

Evaluate the overall status of residents through observations and interviews to complete an accurate MDS.
Complete the MDS on a timely basis as per state and federal regulations.
Input the MDS into the computer and transmit as per state and federal requirements.
Print and complete or assist with the completion of the C.A.T. and C.A.A.
Work with the interdisciplinary team to complete the care plan after the C.A.T. and C.A.A. are completed.
Update or assist with updating care plans as needed.
Communicate with residents, family members, other disciplines, and health care professionals regarding resident status.
Be capable of working the floor if the need arises.
Check the 24-Hour Report daily to be aware of changes in residents’ conditions and/or plans of treatment for potential Significant Change MDS completion.
Keep all departments (Nursing, Social Services, Activities, Dietary and Therapy) aware of the MDS schedule.
