About The Position
John Knox Manor Nursing Home is seeking an experienced, knowledgeable, organized, and compassionate MDS Coordinator to join their team. This role is crucial in ensuring residents receive the care they deserve and plays a key role in accuracy, compliance, and quality resident care.
Requirements
- Previous MDS Coordinator experience in a Skilled Nursing Facility
- Strong knowledge of MDS 3.0 / RAI processes
- Understanding of Medicare, Medicaid, and SNF reimbursement requirements
- Excellent organization skills and attention to detail
- Strong communication and leadership skills
- A team player who is committed to resident-centered care
Responsibilities
- Coordinate and oversee the MDS assessment process
- Ensure assessments are accurate, timely, and compliant with current CMS requirements
- Collaborate closely with Nursing, Therapy, Social Services, Dietary, and other interdisciplinary team members
- Lead and facilitate care plan development and coordination
- Monitor documentation and identify opportunities to improve clinical outcomes
- Support regulatory compliance and survey readiness
- Help ensure the interdisciplinary team is working together toward the best outcomes for our residents
- Maintain certs/recerts
- Initiate death certificates
