The MDS Coordinator plays an important clinical and regulatory role within our skilled nursing community. This position is responsible for keeping the MDS process moving—from assessments and documentation review to interdisciplinary coordination, care planning, reimbursement, and survey readiness. We’re looking for someone who enjoys having ownership of their work, can confidently manage deadlines, and isn’t afraid to ask questions or follow up when something is missing. You’ll work independently while partnering closely with nursing leadership, therapy, social services, and other members of the interdisciplinary team. This is a deadline-driven and detail-intensive position. There will be days when you are balancing multiple assessments, reviewing clinical documentation, tracking down missing information, responding to changes in resident status, and managing competing deadlines. The right person for this position will find satisfaction in bringing organization to that process and making sure it gets done accurately and on time. In return, Eastcastle Place offers flexibility that can be difficult to find in an experienced nursing position, including 28–40 hours per week, a Monday–Friday schedule, flexible start and end times, and the opportunity to take meaningful ownership of the MDS process.
