MDS Coordinator (RN) – Hillcrest Health & Rehab Location: Hillcrest Health & Rehab – Bellevue, NE Address: 1702 Hillcrest Drive, Bellevue, NE 68005 Schedule: Full-Time Are you an experienced MDS nurse who thrives on accuracy, collaboration, and making sure the clinical story is captured correctly? Hillcrest Health & Living is looking for an experienced, detail-oriented MDS Coordinator (RN) to join our clinical team at Hillcrest Health & Rehab in Bellevue, NE. Hillcrest Health & Rehab provides short-term rehabilitation, skilled nursing, and long-term care in a supportive, resident-centered environment. Our interdisciplinary team works together to promote strong clinical outcomes, accurate documentation, regulatory compliance, and exceptional care for the residents and guests we serve. As our MDS Coordinator, you’ll support that mission by ensuring assessments accurately reflect each resident’s needs, strengthening care planning, and helping our clinical teams maintain the documentation and compliance needed to deliver excellent care.

• Confident working across multiple disciplines
• Understand that accurate MDS assessments aren't simply about meeting a deadline—they help tell the resident's clinical story and directly support appropriate care planning and reimbursement.
• Experienced RN with MDS expertise
• Values accuracy, accountability, collaboration, and resident-centered care

Responsibilities

• Lead and coordinate the MDS assessment process while partnering closely with nursing, therapy, social services, dietary, activities, and leadership.
