This position is responsible for coordinating the interdisciplinary team in the timely completion of assessments, utilizing the MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs), and communicating ARD selections to the IDT. The role also involves accomplishing assigned MDS sections, fulfilling CAAs, completing Care Plan items, implementing monthly infection tracking logs, concluding UDAs, finalizing diagnosis coding, confirming skilled physician's certification, and monitoring documentation to support MDS coding. Additionally, the MDS Coordinator participates in skilled utilization and State Case Mix management, provides teaching and training for MDS item completion to interdisciplinary team members, and mentors CNA staff on Nursing ADL documentation. The role requires signing off on the Minimum Data Set (MDS) for each resident.
