The MDS Coordinator is responsible for the coordination of the facility’s Resident Assessment Instrument (RAI) process in accordance with state and federal regulations. This role involves the accurate completion of all MDS assessments and supporting clinical documentation, implementation and evaluation of resident care plans, and leading interdisciplinary care plan meetings. The MDS Coordinator also audits medical records, provides education to staff, and collaborates with health plans and physicians to ensure insurance approvals and eligibility criteria are met. They act as a primary resource for problem-solving related to the SNF prospective payment system and quality reporting program.
