About The Position
Skylake Post Acute is hiring an experienced Full-Time MDS Coordinator (RN/LPN). As one of the largest skilled nursing facilities (SNF) in Colorado, Skylake Post Acute is the top choice for rehabilitation and 24-hour skilled nursing care. The MDS Coordinator will be responsible for collecting MDS documents within mandated time frames, comparing and correcting data, and filing MDS documents while maintaining patient confidentiality.
Requirements
- Current RN/LPN License
- Experience as a MDS Coordinator utilizing 3.0
- Experience in a SNF environment
Responsibilities
- Collects MDS documents within mandated, established and expected time frames.
- Compares data entered against the original MDS document; makes corrections as needed.
- Files the MDS documents in residents' charts.
- Maintains confidentiality of all patient care information.
