Experienced MDS Coordinator - (Full-time 40 hrs.)
About The Position
Moziac Senior Life is seeking an experienced MDS Coordinator for a full-time, 40-hour per week position. This role offers the opportunity to make a difference in a family-like environment where employees are appreciated. The MDS Coordinator will be responsible for all MDS assessments, including initial admissions, quarterly assessments, change of condition assessments, and Medicare/managed Medicare assessments in a Long Term Care setting. This position works collaboratively with Nurse Managers and the Interdisciplinary Team (IDT) to ensure care plans accurately reflect residents' ongoing conditions and to ensure all Medicare residents are skilled.
Requirements
- Graduate of a school of professional nursing.
- Strong clinical background in nursing.
- Head nurse, Care planning and MDS experience.
- Fluency RN State of Connecticut.
- CPR certification/IV-line management 3.0 certification.
Nice To Haves
- BSN preferred.
Responsibilities
- Responsible for all MDS's including the initial admission, quarterly, change of condition, Medicare and managed Medicare in Long Term Care.
- Works collaboratively with Nurse managers and IDT to ensure that Care plans reflect the residents' condition on an ongoing basis.
- Ensure that all Medicare residents are skilled.
