About The Position

First Community Village Senior Living Campus in Columbus, OH is seeking a Full Time MDS Coordinator (RN or LPN). First Community Village is an independent living, assisted living, and memory care community operated by National Church Residences, a leading provider of senior housing and healthcare services. The MDS Coordinator is responsible for completing MDS assessments, including admission assessments, quarterly and significant change updates, annual reviews, CAAs, and electronic submissions for all residents. This role also facilitates plan of care conferences and prepares individualized care plans.

Requirements

  • Graduate of an accredited school (LPN preferred) or vocational school of practical nursing.
  • Training in restorative techniques and in their practical application.
  • Exceptional written, verbal, analytical, leadership, and interpersonal skills.
  • Current Ohio nursing license in good standing.

Responsibilities

  • Completing the MDS and admission assessments, quarterly and significant change updates, annual reviews, CAAs, and electronic submission for all residents in the center.
  • Auditing and coordinating MDS, CAA Summaries, and Plan of Care with the Interdisciplinary Team.
  • Responsible for ICD9 coding.
  • Responsible for Physician Certification.
  • Responsible for Insurance authorizations/updates.
  • Developing a thorough working knowledge of RUGS III/IV scoring process for accurate resident scoring.
  • Setting MDS ARD to maximize assessment scores.
  • Addressing concerns and resident issues with family members/caregivers.
  • Developing a working knowledge of center assessment software.
