About The Position

Providence Healthcare of Thomaston is seeking a Full-Time MDS Coordinator (RN or LPN) to manage the Resident Assessment Instrument (RAI) process for both short-term rehabilitation and long-term care units. This role involves ensuring compliance with accuracy, attestation, and timely completion of resident assessments according to Medicare, Medicaid, OBRA, and other payor program requirements. The MDS Coordinator is responsible for ensuring appropriate documentation to support services provided and assessment accuracy, and for effective communication with the interdisciplinary team. The position requires the ability to collaborate effectively with residents, families, and staff.

Requirements

  • Must be a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
  • Graduate of an approved school for Practical or Vocational Nursing.
  • Currently licensed by the State Board of Nursing.
  • All new hires must be COVID-19 vaccinated or have a medical or religious accommodation before starting.

Nice To Haves

  • MDS and RAI experience is preferred.
  • Experience in geriatric or rehabilitative Nursing is preferred.

Responsibilities
