Pines Village is looking for a motivated and team-oriented Full TIme , RN, MDS Coordinator to join our team. As a member of our team, the MDS Coordinator will need to work Full TIme in office Monday - Friday and be available for the nursing on call schedule  once a month.

Requirements

• Must have experience with MDS/PDPM process for Sub-Acute and LTC residents
• Must have experience operating Point Click Care for assessments and EMR
• Must have good communication skills in order to work effectively and keep accurate, detailed records
• Ensure all MDS are in compliance with State and Federal regulatory requirements
• RN (REquired)
• MDS: 1 year minimum (Required)

Nice To Haves

• RAC-Certified (Preferred)

Responsibilities

• Collaborate with facility’s staff to select the correct assessment of our residents
• Must have good communication skills in order to work effectively and keep accurate, detailed records
• Ensure all MDS are in compliance with State and Federal regulatory requirements
• Participate in developing/updating/reviewing/completion of individualized Care Plans
