MDS Coordinator RN
About The Position
We are currently searching for a MDS Coordinator to join our friendly, caring and supportive team. Rehabilitation Center of Independence West Campus is rapidly growing and our team is looking to invest in a MDS Coordinator by providing opportunities to further your career and with the tools and encouragement you need to succeed. As MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at Rehabilitation Center of Independence West Campus by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.
Requirements
- Unencumbered State of Iowa R.N. License
- Graduate of an accredited nursing program
- CPR Certification
Nice To Haves
- Prior experience as a MDS Coordinator
- One (1) year of experience as an R.N. in a Skilled Nursing Facility setting
- One (1) year of experience as a C.N.A.
Responsibilities
- Developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals
- Performing resident assessments
- Assisting in the discharge process
