About The Position

MDS Coordinator | RN at Timbercreek Rehab and Healthcare Center. Tutera Senior Living & Health Care is seeking health care rockstars to join their team. The MDS Coordinator | RN is responsible for the timely and accurate completion of the RAI process and care management process from admission to discharge, in accordance with company policy, Federal, State, and Certification guidelines. This may include Minimum Data Set, discharge, and admission tracking. With direction from the Director of Nursing and VP of Clinical Reimbursement, the role may also coordinate information systems operations and education for the clinical department.

Requirements

• Currently licensed as a Registered Nurse (RN) in the state in which you are practicing
• Professional image in both appearance and behavior
• Excellent written and oral communication skills
• Knowledge of the care planning process required

• Prior MDS experience highly preferred
• Minimum of one year of experience in a LTC/SNF/AL/MC setting highly preferred
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement highly preferred
• Must possess a thorough understanding of the Quality Indicator process, OBRA regulations, and Minimum Data Set

Responsibilities

• Timely and accurate completion of the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge, and admission tracking, etc.
• Coordinate information systems operations and education for the clinical department (with direction from the Director of Nursing and VP of Clinical Reimbursement).
