About The Position
The MDS Coordinator will conduct a comprehensive assessment (Minimum Data Set Questionnaire) of all Participants prior to enrollment and on an annual basis according to State Guidelines, will input information into the MDS into the State OPTS system and develop or update the Participant Plan of Care in the electronic medical record.
Requirements
- RN or LPN license
- Experience with Minimum Data Set (MDS) assessments
- Familiarity with State Guidelines for assessments
- Proficiency in inputting data into the State OPTS system
- Ability to develop and update Participant Plans of Care
- Experience with electronic medical records
