As an MDS Coordinator, you will work collaboratively with an interdisciplinary team to assess residents' needs and optimize their care plans.
Requirements
- Active RN License in Oregon (or ability to obtain)
- Strong interpersonal communication and teamwork skills
- Experience with the RAI process and Case Management
- Willingness to travel frequently across Oregon
Responsibilities
- Facilitate the pre-admission process, securing essential information for MDS optimization
- Review resident care and set ARDs to ensure accurate Prospective Payment System MDS Assessments
- Maintain state and federal compliance with timely and precise resident assessments
- Accurately code MDS data using information from staff interviews, medical records, and resident observations
- Oversee MDS schedules and assist with other assigned responsibilities
