About The Position

The MDS Coordinator-LVN-PRN is responsible for assessing residents, completing the Minimum Data Set (MDS), and implementing health care plans. This role involves coordinating rehabilitative and restorative care, acting as a liaison between residents, families, and staff, and participating in committees such as Infection Control and Quality Assurance. The position requires a thorough understanding of state and federal guidelines for resident care and assessment.

Requirements

Graduate of an accredited program of nursing education.
Training and Experience in the care of the geriatric patient as evidenced by completed courses and clinical practice.
L.V.N. with a current license in the state of Texas.
MDS certification.
Proven ability to communicate effectively.
Knowledge of general nursing theory and practice, particularly in the area of geriatrics.
Must be knowledgeable of the survey process and adhere to current state and federal guidelines concerning the delivery of resident care and the nursing assessment process.

Responsibilities

Reviews each preadmission application for residency and, when possible, assesses the applicant in person for level of care designation.
Assures that each resident's admission status is assessed and documented within eight hours of admission.
Completes the Minimum Data Set within fourteen days of admission.
Implements necessary actions for interdisciplinary health care plan within 21 days of admission.
Assists in preparation for and during all surveys.
Demonstrates knowledge and judgment concerning resident care delivery and utilizes the nursing process.
Demonstrates observation skills for obvious and subtle changes in resident's condition.
Retrieves information for changes in status via lab reports and presentation of symptoms as noted in 24 hour reporting.
Documents changes as necessary of M.D.S. and directs changes in health care plan.
Coordinates the provision of service centered around the need for rehabilitative and restorative care on all residents requiring this support.
Supervises the care delivery of the rehabilitative aides on each unit and directs interventions through physician consultations, physical therapy, and nursing assessment.
Acts as liaison between resident and family members and staff.
Is knowledgeable of TILE systems to enhance optimum program adoption for residents and to enhance revenue avenues.
Is an active participant of the Infection Control Committee and Quality Assurance Committee.
Performs resident care audits and makes rounds as necessary to ensure compliance.
Plays an integral part in the health care plan process by bringing knowledge from prior living status and by stressing the need for continuity of care.
Assures through involvement of resident and significant others in the health care plan process that resident's wishes are mirrored in the approaches to be used.
Assures that all goals on Health Care Plan are measurable.
Assists licensed and non-licensed staff in understanding and following the plan of care.
Is available to the Directors of Nursing for a report and to assess changes to physical and psycho-social status of the residents.
