About The Position
As our MDS Coordinator, you'll play a vital role in driving quality outcomes, ensuring assessment accuracy, and supporting exceptional resident care. This position offers the opportunity to combine clinical expertise with leadership while making a meaningful impact on residents, families, and the facility's success. Avalon Care Center Yakima is a 101 bed facility located in sunny Central Washington and offers year-round access to beautiful scenery, outdoor recreation, and adventure.
Requirements
- Current Washington LPN license (or NLC-eligible)
- Previous MDS Coordinator experience
- Strong knowledge of the RAI process and case management
- Strong attention to detail and organizational abilities
- Ability to meet all employment screening requirements
Responsibilities
- Oversee the MDS process and resident assessments
- Ensure accurate, timely MDS completion and regulatory compliance
- Coordinate care planning with interdisciplinary teams
- Support Medicare and Managed Care requirements
- Drive quality outcomes and reimbursement accuracy
- Serve as a key clinical and operational partner within the facility
