About The Position
CarDon is seeking an MDS Assistant Licensed Practical Nurse to support the comprehensive assessment and care-planning process for residents. The role assists with collecting, organizing, verifying, and entering clinical and functional data for accurate Minimum Data Set (MDS) documentation. The assistant works with nursing staff, therapists, and other healthcare professionals to gather information for resident care plans and regulatory compliance. By maintaining accurate records and facilitating communication among care-team members, the position supports resident-centered care, quality outcomes, and compliance with state and federal guidelines.
Requirements
- Current, valid Licensed Practical Nurse (LPN) license in Indiana.
- Basic knowledge of healthcare terminology and clinical documentation.
- Proficiency with computer systems and electronic health records.
- Strong attention to detail and organizational skills.
- Ability to communicate effectively with multidisciplinary teams.

- Experience in a long-term care or healthcare setting.
- Familiarity with MDS assessment processes and regulatory requirements.
- Completion of relevant healthcare or administrative training programs.
- Ability to manage multiple tasks and prioritize workload.

Responsibilities
- Assist in collecting and verifying resident data for MDS assessments.
- Collaborate with nursing and therapy staff to ensure timely and accurate documentation.
- Enter and update resident information in electronic health record systems accurately.
- Support the MDS Coordinator in preparing reports and submitting data to regulatory bodies.
- Maintain confidentiality and comply with healthcare regulations and facility policies.
