About The Position

Responsible for monitoring the health of the mothers, babies, and children living at First Step Home Inc. This role involves initial assessments, arranging medical care, providing health education, and collaborating with various teams to ensure comprehensive client support.

Requirements

  • Graduate of an accredited school of nursing and holder of current state license.
  • LPN or RN
  • Behavioral Health experience
  • CPR certified.
  • CDCA or obtain within 90 days
  • Minimum of six months of recent experience as a professional nurse.

Responsibilities

  • Meet with newly admitted pregnant women and women with children to complete an initial assessment.
  • Notify the medical and clinical team and others of any needs observed during the assessment.
  • Arrange obstetric care for newly admitted maternal clients and pediatric care for children.
  • Meet weekly with pregnant women and children under one to teach/review: feeding, safe sleep, car seat safety, basic maternal health care.
  • Check baby’s weight and vitals each week during their visits.
  • Meet with all children over one year old at least once per month.
  • Facilitate two maternal groups with a focus on fetal development during all trimesters, self-care during pregnancy and caring for baby after delivery etc.
  • Collaborate with case management and the parent coordinator to connect women and children with existing services.
  • Provide Care Coordination services by scheduling appointments for maternal women and children.
  • Collaborate with PPC about appointments, missed appointments and follow up with high risk clinic.
  • Complete Edinburgh assessments and report results to clinical and medical team.
  • Complete grant requirements.
  • Review all client summaries/discharge papers following visits from doctor’s appointments or hospital stays.
  • Collaborate with FSH medical and clinical team for client referral for psychiatric assessment.
  • Collaborate with medical and clinical team with developing trauma informed birth plans.
  • Collaborate with clinical and medical team with supporting our clients to advocate for themselves when they are in the hospital, to ensure they are receiving fair and respectful treatment.
  • Communicate appropriately with outside agencies and maintain professional relationships.
  • Work to attain the goals as written in job performance evaluations.
  • Abide by the standards as set forth in First Step Home’s Compliances Plan and Code of Conduct.
  • Follow all safety rules and safe health practices of First Step Home, Inc.
  • Comply with all agency policies, procedures and directives of immediate supervisor, the President/CEO and the Board of Trustees of First Step Home.
  • Follow the Core Values of First Step Home.
  • Perform other duties as assigned.
