About The Position

The Medication Assisted Treatment Care (MAT) Care Coordinator serves as a vital link between patients, healthcare providers, and community resources to ensure seamless, high-quality care. This role involves assessing patient needs, developing care plans, coordinating services, and advocating for patient well-being. The MAT Care Coordinator collaborates with interdisciplinary teams to improve health outcomes, reduce barriers to care, and enhance patient satisfaction. The MAT Care Coordinator is required to perform duties as outlined in this job description.

Requirements

  • Proficient computer skills, including Microsoft Office (specifically Word and Excel)
  • 2-5 years’ experience in a clinical setting
  • Self-disciplined, energetic, passionate, innovative
  • A team player that can follow a system and protocol to achieve a common goal
  • Highly organized and well-developed oral and written communication skills
  • Demonstrates sound judgment, decision-making and problem-solving skills
  • Able to maintain confidentiality with all aspects of information in accordance with practice, State and Federal regulations
  • Confidence to communicate and outreach to other community health care organizations and personnel
  • 1 year in a Patient-Centered Medical Home clinical setting or knowledge of the Patient-Centered Medical Home initiative
  • 2-5 years’ experience in chronic disease management, case management, utilization management, and adult acute care
  • Other licensed medical professionals who possess the appropriate clinical skills are also eligible.
  • Experience with electronic medical records

Responsibilities

  • Maintain strong communication across care teams and providers to ensure alignment on patient care plans and support all MAT‑related patient needs.
  • Review patient records and create accurate tasks or encounters for the MAT Core Team to support coordinated care.
  • Coordinate patient services with external partners, including behavioral health agencies, therapy providers, IOP programs, inpatient facilities, and community resources.
  • Assist patients in accessing essential social services such as financial support, transportation, housing resources, and community programs.
  • Manage all MAT‑related scheduling, including provider appointments, group visits, urgent patient calls, and ensuring MAT patients are scheduled with both PCP and Behavioral Health.
  • Oversee MAT medication processes by ordering injectable medications, tracking inventory, logging medication stock, maintaining active injectable‑medication lists, and completing all prior authorizations.
  • Maintain accurate patient records by updating the active MAT Core patient list, reviewing SUD assessments from SBHC and uploading them into ECW, and verifying completion of required testing (HIV/HCV) and new‑patient requirements (contracts, bracelets).
  • Monitor patient engagement by tracking no‑show patterns and collaborating with PCPs or the Core Team to determine appropriate follow‑up steps.
  • Coordinate care for MAT OB patients in collaboration with the OB team.
  • Support patient medication management by following standing orders and protocols and ensuring lab results (including Beechtree) are brought to the Core Team for review.
  • Contribute to quality improvement by evaluating clinical processes, supporting resource utilization efforts, and assisting in the development of new tools, forms, and procedures.
  • Participate in Quality Management Committee activities, including data collection, outcomes reporting, clinical audits, and program evaluation.
  • Manage documentation tasks such as scanning and uploading records to the MAT folder and appropriate lab locations and faxing medical records upon receipt of a signed release.
  • For patients expressing suicidal thoughts, provide Access Center information, notify the provider and behaviorist, and create a task/encounter for follow‑up.
