About The Position

The Manager, Practice Transformation serves as Health Choice Network’s (HCN) subject matter expert on quality initiative program requirements, including Patient-Centered Medical Home (PCMH), and supports the design, development, and analysis of reporting software and data tools. This role partners closely with practices to integrate administrative, financial, and clinical systems, driving performance improvements and better patient outcomes.

Requirements

  • Bachelor’s degree in nursing, healthcare administration, public health, or related field.
  • PCMH Certified Content Expert (CCE) certification required
  • Minimum of 5 years of experience in quality improvement, practice transformation, or value-based care within a healthcare or CHC environment.
  • Strong analytical and data interpretation skills with the ability to translate findings into actionable strategies.
  • Familiarity with HRSA quality programs, value-based care models, and population health management tools.
  • Excellent facilitation, communication, and relationship-building skills.

Nice To Haves

  • Experience with Epic or other electronic health record systems preferred.

Responsibilities

  • Lead and guide clinical practices in establishing a planned care model that aligns with HCN’s quality and performance goals.
  • Utilize data and analytic tools to assist clinicians in capturing and interpreting population-based data to support data-driven decision-making.
  • Support practice leadership in developing skills to act on quality metric data through performance management strategies.
  • Facilitate care coordination efforts through population health management tools to identify and stratify patient risk and enhance patient engagement.
  • Ensure practices continue transformative activities that demonstrate measurable improvement on quality and performance measures.
  • Supervise and provide direction to the Practice Transformation Coordinator.
  • Provide customized coaching to clinical sites (e.g., community health centers) to identify site-specific needs, prioritize areas of focus, and allocate resources effectively.
  • Collaborate with the Chief Clinical Officer and Physician Champions to document promising practices and streamline provider workflows.
  • Assess and monitor quality measures, coaching clinic staff on effective performance tracking and improvement.
  • Guide clinic leadership and staff in implementing best practices and overcoming barriers to process improvement.
  • Encourage site leadership to leverage existing data sources to minimize data collection burden and improve reporting efficiency.
  • Partner with practices to develop progress reports on quality, financial, and utilization goals, sharing results and insights across the network.
