About The Position

This job assumes responsibility for strategic planning, direction, operations and overall accountability for Social Service component of the Care Management department; assumes responsibility for creating innovative social service solutions to ensure quality outcomes, achieving shared savings quality metrics and decreasing cost/utilization; independently makes decisions in accordance with successfully achieving the goals of the company's quality objectives, financial objectives, performance objectives and strategic results; and develops and assumes accountability for Social Services budget. Requirements

  • Master's degree in Social Work
  • 7 years of experience in a healthcare or social service setting as a social worker including 2 years supervision or management experience
  • Licensed Clinical Social Worker (LCSW) as verified by the LABSWE
  • Effective verbal and written communication skills and the ability to present information clearly and professionally.
  • Experience with DSM-5 diagnostic criteria and evidence-based treatment modalities.
  • Knowledge of crisis intervention techniques, suicide risk assessment, and trauma-informed care.
  • Ability to exhibit excellent leadership and self-direction, good judgment in handling difficult situations and good organizational, time management, interpersonal and conflict resolution skills.

Nice To Haves

  • Case Management Certification (such as CCM, ACM, CMAC)

Responsibilities

  • Ensures quality of care & patient satisfaction through unit-specific systems and improvements.
  • Manages, develops, retains and inspires an engaged workforce.
  • Assists with research, education & training to support cutting edge patient care and services.
  • Ensures operational profitability with preliminary budgets, performance data, system reporting, and inventory management.
  • Supports long-term strategic business plans through department-specific business planning and special projects.
  • Manages the physical & technological infrastructure and supports organizational structures.
  • Remains knowledgeable on current federal, state and local laws, accreditation standards or regulatory agency requirements that apply to the assigned area of responsibility and ensures compliance with all such laws, regulations and standards.
