About The Position

The Manager of Homecare Under the supervision of the Director of Clinical Services oversees the day-to-day operations and operational effectiveness of the Home Care department to ensure high quality, safe, effective care is provided in conformance with policies, procedures and standards and all governing state and federal regulations. Participates in the Interdisciplinary Team (IDT) Meeting as required, acting as the Home Care representative

Requirements

  • Minimum requirement of C.N.A. certification
  • Cardiopulmonary Resuscitation (CPR) certification required
  • Bachelor’s Degree or equivalent education and experience
  • A minimum of one year’s experience in working with the frail or elderly population required
  • Minimum of two (2) year’s management experience in a hospital, nursing home or community-based setting

Nice To Haves

  • Current State Specific certification as a Licensed Practical Nurse, Registered Nurse, Occupational Therapist, Physical Therapist or other licensed professional preferred.
  • Home health experience preferred.

Responsibilities

  • Directly responsible for the operational oversight of all Personal Care Supervisor(s), Home Care Scheduler & Home Care aides including schedule management, and overall operational effectiveness of the department.
  • In coordination with the Director of Clinical Services, performs orientation, initial and annual competencies, and is responsible for evaluating performance of all home care staff and schedulers.
  • In coordination with the Director of Clinical Services, performs quarterly supervisory visits in the home of each participant receiving home care services. At least once per year, the Manager of Home Care educates home care aides on best practices in Home and Day Center caregiving practices.
  • Prioritizes utilization of homecare staffing needs to reflect actual enrolled participant’s homecare needs.
  • Performs initial, semi-annual & change of status assessments of participants’ in their home environment to determine their individual needs and coordinates plan of care with care-giving resources.
  • Coordinates with the Interdisciplinary Team to develop plan of care for each participant.
