Requirements

  • Active Registered Nurse (RN) license or other clinical licensure relevant to the role
  • Minimum of 3-5 years of clinical experience in a healthcare setting
  • At least 2-3 years in a leadership or supervisory role
  • Experience with chronic care management and transitional care management
  • Familiar with telehealth care delivery and home-based patient care
  • Strong leadership, communication, and interpersonal skills
  • Data analysis and performance management expertise
  • Ability to foster a collaborative and team-oriented environment
  • Proficiency in healthcare software and Electronic Health Records (EHR) systems
  • Knowledge of healthcare regulations, quality standards, and compliance requirements

Responsibilities

  • Collaborate with other clinical leaders to develop care guidelines, pathways, and criteria aimed at improving patient interactions and achieving better quality outcomes.
  • Provide expert insight on delivering care in patients' homes and through telehealth platforms.
  • Provide leadership and guidance to clinical support staff, including RNs and Care Navigators, to ensure they perform their duties effectively and deliver high-quality care.
  • Oversee recruitment, hiring, and management of the clinical support team.
  • Ensure the successful onboarding of new staff members, providing the necessary training and support.
  • Management of chronic care management and transitional care management initiatives, including outreach, assessment, and intervention for patients.
  • Monitor and evaluate the effectiveness of care management strategies and make recommendations for continuous improvement.
  • Monitor clinical staff performance, conduct regular performance evaluations, and provide constructive feedback.
  • Address performance issues in a timely and supportive manner and promote continuous professional development for team members.
  • Hold monthly one-on-one meetings with staff members to discuss performance, provide feedback, and address concerns.
  • Use data from Key Performance Indicators (KPIs) to make informed decisions and drive improvement initiatives.
  • Review outcome data and participate in performance improvement activities to enhance patient outcomes.
  • Work cross-functionally with other departments to design and implement workflow efficiency initiatives.
  • Ensure that clinical processes are streamlined and aligned with organizational goals to improve operational effectiveness.
  • Approve Paid Time Off (PTO) requests and oversee time clock management to ensure proper staffing and operational continuity.
  • Hold weekly clinical meetings to discuss updates and review best practices.
  • Perform various clinical and administrative tasks as needed, ensuring the smooth operation of the clinical department.
