About The Position

The Manager of Care Management is responsible for creating safe, healthy environments that support the work of the care management team to improve patient health outcomes, patient engagement, and patient experience while minimizing cost to patients and families and SHC. The role is influential in creating a professional environment and fostering a culture where interdisciplinary team members are able to apply the SHC FOCUSEDTM Model of care and Care Management guiding principles. Provides service and supervision in the practice of Care Management to ensure processes optimize (a) population health, (b) management of care transitions and (c) deployment and adherence to plan of care. Demonstrates knowledge of and expertise in the philosophy and standards of professional Care Management, assists with the application of these to diverse populations with special consideration of age and cultural needs, as well as SHC requirements. Requirements

  • BSN
  • RN License in state of practice OR Social Worker License in state of practice
  • If SW License Eligible, must sit for exam within 6 months of hire, or upon eligibility
  • 2+ years experience in a Healthcare (medical / surgical) setting
  • 2+ years Care Management
  • Case Management Certification (CCM, ACM, or ANCC) required. If not currently certified, certification by CCM, ANCC, or ACM will be required within one year of hire, or upon eligibility.

Nice To Haves

  • MSN or Master of Social Work
  • 2+ years Pediatrics experience
  • 3+ years management experience in a supervisory/leadership role
