About The Position

The Manager, Health Navigation will oversee and manage a team of Health Navigators across SAC Health. This team will support all SAC Health patients needing assistance with Social Determinants of Health (SDOH), application processes, health education, insurance eligibility, care coordination, and other needs identified by providers or other healthcare team members. Reporting directly to the Director of Quality Care, this manager will work closely with other quality managers, departments, providers, and staff to ensure the delivery of optimal health navigation services. As the manager of the team, this individual will be responsible for ensuring effective organization, training, support, and communication within the team, as well as fostering collaboration with the broader Quality Department.

Requirements

  • Master's degree in social work, Marriage and Family Therapy, or related field required.
  • Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapy (LMFT) required.
  • Current CPR/BLS certification (must be American Heart Association or Red Cross accredited program).
  • As a requirement of this position, you must receive EPIC certification for the module you have been hired into.
  • Valid California driver's license, and auto insurance.
  • 5+ years working within a community-based setting and 1 year of supervisory level experienced required.
  • Must be proficient in MS Office Suite: Word, Excel, PowerPoint, Outlook.
  • Must be able to use widely support internet browsers.
  • Must have the ability to use variations of electronic health records and other various databases.
  • Able to relate and communicate positively, effectively, and professionally with others.
  • Be assertive and consistent in following and or enforcing policies.
  • Work calmly and respond courteously when under pressure.
  • Lead, supervise, teach, and collaborate.
  • Accept direction and be able to communicate effectively in English in person, in writing, and on the telephone.

Responsibilities

  • Oversee and manage clinical situations that necessitate the supervision of a licensed healthcare provider, including the management of urgent or emergency care scenarios, and handling crisis situations that demand immediate attention and specialized intervention.
  • Ensure documentation records meet compliance and regulatory requirements, including providing clinical sign-off as necessary.
  • Ensuring appropriate clinical oversight and prompt follow-up and intervention in the delivery of care management services.
  • Provide clinical supervision to up to eight Associate Clinical Social Workers (ACSWs) at a time, in accordance with BBS guidelines.
  • Responsible for the daily supervision and oversight of Health Navigation staff.
  • Responsible for the hiring, training, and development of staff.
  • Accountable for the productivity and performance of the Health Navigation team, based on departmental needs and the volume of incoming referrals.
  • Collaborate with other care management programs, both internal and external, to ensure staff have the tools necessary to provide services to patients.
  • Maintain a strong working knowledge of available community resources and guide staff in identifying and connecting patients with the most relevant and effective support services.
  • Work closely with leadership, departmental teams, and staff to identify areas for enhancement, implement evidence-based strategies, and monitor progress toward achieving measurable outcomes.
  • Work with the Director, Quality Care as well as the other identified managers in the department to ensure excellent patient care as well as collaboration between all Quality Department efforts.
  • Responsible for collecting and analyzing data on completed tasks and projects, as well as providing regular updates and comprehensive reports to leadership.
  • Responsible for engaging clinical leadership teams to develop, implement, and facilitate provider-driven referrals for SDOH or Care Management related needs.
  • Responsible for the generation of new ideas, workflows, processes, and policies that improve quality, efficiency, and effectiveness within the program and support departmental and organizational goals.
  • Responsible for time and attendance oversight: scheduling, timecards and disciplinary action.
  • Responsible for the implementation and execution of any grants assigned to the Health Navigation division within the Quality Department.
  • May provide cross coverage. Travel to other clinics as necessary; must have a reliable vehicle, valid driver's license, and auto insurance.
