About The Position

The Community Health Advocate team is responsible for improving health outcomes and reducing barriers to care for Blue Shield Promise Health Plan members through field-based outreach, care navigation, community engagement, and support services that address social drivers of health. The Manager, Community Health Advocate (CHA) will report to the Sr. Director, Community Programs. In this role, you will lead a team of field-based Community Health Advocates who conduct home visits, accompany members to medical appointments, perform social needs assessments, connect members to community resources, and provide hands-on support to improve access to care. You will be responsible for the day-to-day leadership, performance management, coaching, and development of the CHA team while ensuring high-quality member engagement and service delivery. Requirements

  • Requires a bachelor's degree or equivalent experience
  • Requires at least 7 years of prior relevant experience, including 3 years of people management experience gained as a team leader, supervisor, project manager, or program manager
  • Requires experience working with Medi-Cal members and other vulnerable, underserved, hard-to-reach populations
  • Requires experience with field-based programs such as home visits, community outreach, and care navigation.
  • Demonstrated experience managing operational performance through metrics, reporting, and continuous improvement initiatives
  • Strong knowledge of social drivers of health and community resource networks
  • Excellent oral and written communication skills with the ability to influence and collaborate across cross-functional teams and external partners
  • Ability to build strong relationships with health plan members, providers, community organizations, and key stakeholders
  • Valid California Driver's License and ability to travel throughout Los Angeles and San Diego counties to support field operations and staff development

Responsibilities

  • Lead and develop a team of field-based Community Health Advocates (CHAs) serving Blue Shield Promise members throughout Los Angeles and San Diego counties
  • Provide coaching, mentoring, performance management, and professional development to support a high-performing, member-centered team
  • Establish operational goals, performance expectations, staffing plans, and workflows aligned with departmental objectives and member outcomes
  • Conduct regular field observations and site visits to ensure quality service delivery, provide real-time coaching, and support complex member cases
  • Oversee care navigation, member outreach, and community engagement activities that address social drivers of health and improve access to healthcare and community resources
  • Assist with the development, implementation, and evaluation of programs designed to improve member engagement, health outcomes, care quality, and cost effectiveness
  • Ensure timely, accurate, and compliant documentation of member interactions, assessments, referrals, and outcomes within applicable systems
  • Develop and maintain standard operating procedures, policies, and best practices for the CHA team
  • Monitor operational and program performance through key performance indicators, dashboards, and reporting, and implement continuous improvement initiatives
  • Collaborate with internal departments, providers, community-based organizations, and external stakeholders to enhance member access to services and strengthen community partnerships
  • Communicate program performance, operational priorities, and member needs to leadership and key stakeholders
