About The Position

The Community Health Advocate team is responsible for improving health outcomes and reducing barriers to care for Blue Shield Promise Health Plan members through field-based outreach, care navigation, community engagement, and support services that address social drivers of health. The Manager, Community Health Advocate (CHA) will report to the Sr. Director, Community Programs. In this role, you will lead a team of field-based Community Health Advocates who conduct home visits, accompany members to medical appointments, perform social needs assessments, connect members to community resources, and provide hands-on support to improve access to care. You will be responsible for the day-to-day leadership, performance management, coaching, and development of the CHA team while ensuring high-quality member engagement and service delivery. Requirements

  • External hires must pass a background check/drug screen.
