Requirements
- Graduate from a recognized, accredited school of nursing or Bachelor’s Degree in Social Work, Healthcare Administration, or other related field
- 5 years If RN: Need combination of Nursing and Care Management exp in a hospital setting. Experience must include leadership/Management exp in acute care setting
- 5 years If Social Worker: clinical Social Worker experience, including leadership/management experience in an acute care setting
- If candidate is a nurse, then a current registered nurse (RN) license in the state of Georgia (or compact state) Upon Hire
- If candidate is a social worker, then a current unrestricted licensed master social worker (LMSW) licensure in the state of Georgia Upon Hire

Responsibilities
- Provides leadership, coaching and mentoring of the Care Management department professional and administrative staff in order to support and ensure standards of practice for service delivery and organizational performance strategies are consistent with PHC's Mission, Vision and Values.
- Provides Care Management services for hospital patients as appropriate to guide care facilitation, coordination, and transitions of care.
