About The Position

We are seeking an experienced Care Management District Manager to oversee the operational performance, implementation, and continued success of our care management service lines across multiple clinics and districts. This role partners closely with Business Administrators, Medical Directors, Clinical Leadership, Population Health, Revenue Cycle, Quality, Compliance, and Care Management Supervisors to ensure operational excellence, regulatory compliance, provider engagement, and strong financial and quality outcomes. The District Manager oversees operations for programs including: Chronic Care Management (CCM), Principal Care Management (PCM), Advanced Primary Care Management (APCM), Transitional Care Management (TCM), Behavioral Health Integration (BHI), Collaborative Care Management (CoCM), Remote Patient Monitoring (RPM), Remote Therapeutic Monitoring (RTM), and Other current and future CMS-approved care management programs.

Requirements

  • Bachelor's degree in Healthcare Administration, Nursing, Business Administration, Public Health, or a related field.
  • 5+ years of progressive healthcare operations leadership experience.
  • Experience implementing and managing CMS care management programs.
  • Experience in ambulatory care, value-based care, population health, or primary care operations.
  • Strong knowledge of CMS reimbursement methodologies, quality reporting, and regulatory requirements.

  • Master's degree (MBA, MHA, MPH, MSN, or related field).
  • Experience leading multi-site healthcare operations.

Responsibilities

  • Execute the strategic vision established by the Director of Care Management.
  • Lead implementation and adoption of new care management programs, workflows, and operational initiatives.
  • Coordinate implementation activities with clinic leadership and operational teams.
  • Monitor implementation progress and resolve operational barriers.
  • Provide feedback and recommendations to support continuous improvement.
  • Ensure consistent execution of approved workflows, SOPs, and organizational policies.
  • Conduct routine operational reviews to evaluate productivity, workflow compliance, patient engagement, and program performance.
  • Monitor KPIs, KRIs, dashboards, and operational scorecards.
  • Develop and implement corrective action plans to address operational gaps.
  • Promote consistency and standardization across assigned clinics and districts.
  • Build strong relationships with providers, Medical Directors, Business Administrators, Care Management Supervisors, and clinic leaders.
  • Drive provider participation and engagement in care management programs.
  • Support change management, workflow optimization, and operational coaching.
  • Resolve issues affecting provider efficiency, patient access, and care coordination.
  • Drive achievement of operational, productivity, enrollment, reimbursement, and financial goals.
  • Monitor documentation quality and revenue cycle performance.
  • Identify opportunities to improve efficiency and optimize resources.
  • Support Quality Assurance (QA) and Quality Control (QC) initiatives.
  • Conduct operational audits and monitor regulatory compliance.
  • Develop corrective action plans for compliance and operational deficiencies.
  • Escalate significant regulatory, operational, or patient safety concerns when appropriate.
  • Coach, mentor, and support Care Management Managers and operational leaders.
  • Assist with onboarding, competency validation, leadership development, and continuing education.
  • Foster a culture of accountability, collaboration, continuous improvement, and patient-centered care.
  • Monitor district performance against organizational goals and quality metrics.
  • Conduct routine business reviews with clinic leadership.
  • Support implementation of new technologies, EHR workflows, automation tools, dashboards, and patient registries.
  • Assist with workflow validation, user acceptance testing (UAT), and technology adoption.
