Manager, Care Coordination (RN), Emergency Department. Requirements
- Active NY Registered Nurse license
- 2-3 years experience in health care related discipline and/or discharge planning or community healthcare experience
- Excellent interpersonal skills
- Ability to respond to short deadlines

Responsibilities
- Discuss discharge needs with Physicians and Social Workers
- Partner with unit nurses to communicate with patients/family about admission and plan of care
