About The Position

Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strengths and choices. Our Delaware County Program is seeking a Manager for our Blended Case Management Team. The Manager coordinates all operations of the Coordinated Evening Outreach, Chronic Homeless Initiative, and Emergency Code Outreach Initiatives. This position represents the Agency by making presentations to key City stakeholders. The ideal candidate will be able to ensure compliance with all applicable contract regulations and agency policies and procedures. Represents the agency and participate in all activities related to outreach activities of the city. Cooperate with all efforts of the city’s Office of Adult Services/Office of Emergency Shelter and Services to coordinate and monitor activities of these programs. Assists with analysis and identification of service delivery gaps, changes and problems, under the guidance of the Associate Director. Review and audit client’s file ensuring that documentation is complete and clinically sound. Maintain and coordinate program documentation to include assessments, service plans and progress notes. Participates in the identification and assessment of staff training and development needs. May assist with course design. Develops and maintain schedules for staff of Coordinated Evening Outreach and Sidewalk Ordinance programs. Also coordinate schedules for Emergency Code Outreach Project. Represents the agency and participate in all activities related to outreach activities of the city. Cooperate with all efforts of the city’s Office of Adult Services/Office of Emergency Shelter and Services to coordinate and monitor activities of these programs. Maintains a liaison with all cooperating agencies/facilities and advocacy/support groups with whom placements made.

Requirements

  • B.A. degree in Human Services/Social Sciences, Healthcare or a related field, and 4 years of experience in the MH/MR or related field, at least 1 years of supervisory experience; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.
  • 2-3 years substance abuse experience necessary
  • Detail oriented
  • Superior assessment skills with comprehensive knowledge behavioral health disabilities, recovery principles and psychosocial rehabilitation practices.
  • Excellent communication, and organizational skills.
  • Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with clients.
  • Must be available for local travel and possess a valid driver's license.
