About The Position

This role enables associates to work virtually full-time, except for required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. Alternate locations may be considered if candidates reside within a commuting distance from an office. Please note that per our policy on hybrid/virtual work, candidates not within a reasonable commuting distance from the posting location(s) will not be considered for employment, unless accommodation is granted as required by law. The Managed Care Coordinator will be responsible for performing telephonic or face-to-face history and program needs assessments using a tool with pre-defined questions for the identification, evaluation, coordination and management of member's program needs.

Requirements

  • BA/BS degree and a minimum of 1 year of experience working directly with people related to the specific program population or other related community-based organizations; or any combination of education and experience which would provide an equivalent background.

Nice To Haves

  • BA/BS degree field of study in health care related field preferred
  • 4-6 years’ experience in the Behavioral Health Field
  • Travels to worksite and other locations as necessary
  • Community Health Worker certification (Qualified Mental Health Professional – QMHP)

Responsibilities

  • Using tools and pre-defined identification process, identifies members with potential clinical health care needs (including, but not limited to, potential for high-risk complications)
  • Coordinates those members’ cases with the clinical healthcare management and interdisciplinary team in order to provide care coordination support. The process does not involve clinical judgment.
  • Manages non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities, to ensure cost effective and efficient utilization of services.
  • Establishes short- and long-term goals in collaboration with the member, caregivers, family, natural supports, and physicians.
  • Identifies members that would benefit from expanded services.
