About The Position

Transition Home Healthcare is an elite home health agency serving Houston, Texas, and surrounding areas. The LVN provides home health nursing care, including visits to patients’ homes, documentation, communication with the healthcare team, and education for patients and families. The role is available as PRN or full-time and covers the North, Northwest, Northeast, Southwest, and Inner Loop service areas. Requirements

• Hold a valid, current LVN license in Texas or a compact state.
• Have at least one year of experience in a home health setting.
• Know home health regulations and general healthcare compliance.
• Have excellent communication and people skills for interacting with patients, families, and healthcare team members.
• Demonstrate a compassionate and empathetic demeanor and commitment to patient-centered care.

Nice To Haves

• Experience with HCHB EMR; preferred but not required.

Responsibilities

• Collaborate with the home healthcare team to develop and implement individualized care plans.
• Complete home care nursing visits.
• Prepare timely, complete, and accurate clinical notes.
• Communicate with healthcare providers and other care team members.
• Educate patients and their families about health conditions, treatment plans, and self-care techniques.
• Follow agency policies, procedures, and regulatory requirements related to home health care.
