About The Position
Transition Home Healthcare is an elite home health agency serving Houston, Texas, and surrounding areas since 2010. The agency is dedicated to delivering high-quality patient care, supporting families, empowering team members, and enriching the community. The LVN provides home health nursing visits and collaborates with the home healthcare team to develop and implement individualized care plans. The role also involves documenting care accurately and promptly, communicating with healthcare providers and care team members, educating patients and families, and following agency policies and applicable home health regulations. The position is PRN and covers North, Northwest, Northeast, Southwest, and Inner Loop service areas.
Requirements
- Valid and current LVN license in Texas or a compact state.
- At least one year of experience in a home health setting.
- Knowledge of home health regulations and general healthcare compliance.
- Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare team members.
- Compassionate and empathetic demeanor, with a commitment to patient-centered care.
Nice To Haves
- Experience with HCHB EMR; preferred but not required.
Responsibilities
- Collaborate with the home healthcare team to develop and implement individualized care plans.
- Complete home care nursing visits.
- Prepare timely, complete, and accurate clinical notes.
- Communicate with healthcare providers and other care team members.
- Educate patients and their families about health conditions, treatment plans, and self-care techniques.
- Adhere to agency policies, procedures, and regulatory requirements related to home health care.
