About The Position

Houston Hospice is seeking PRN LVNs for its In-patient Unit. As the oldest and largest independent nonprofit hospice in the Greater Houston area since 1980, Houston Hospice is dedicated to providing uncompromising, compassionate, end-of-life care to patients of all ages and their families across 13 counties. The organization focuses on caring, not curing, and fosters a team-oriented environment where skilled healthcare professionals bring comfort, dignity, and peace to patients. Care is provided in homes, long-term care facilities, and the inpatient unit at the Texas Medical Center. The LVN on the IPU team will provide hands-on bedside care, monitor symptoms, and offer support to patients and families. This role involves collaboration with a multidisciplinary team including Physicians, RNs, Social Workers, Chaplains, Hospice Aides, Volunteers, and Bereavement Counselors.

Requirements

Graduate of an accredited L.V.N. school or through reciprocity.
Currently licensed as an L.V.N. in the State of Texas.
One year of experience as an L.V.N.
Current certification in basic life support.
Ability to cope with family emotional stress and tolerant of individual lifestyles.
Good organizational skills are required to handle a variety of tasks simultaneously.
Good communication skills, both oral and written.
Effective interpersonal skills.
Ability to accommodate multi-faith beliefs and backgrounds of patients/families.
Ability to function as part of an interdisciplinary team.
Must have reliable transportation to work assignments.

Nice To Haves

Home health, hospice, or oncology experience preferred.
Knowledge/proficiency in applicable federal/state laws governing hospice licensure is preferred.

Responsibilities

Provide care for patients and families according to an individualized plan of care and by agency policies and regulations that define the scope of practice.
Participate in shift assessments, administration of medications, and bedside care.
Assist patient/family in learning appropriate care techniques.
Assist and support the R.N. and/or physician in performing specialized duties related to end-of-life care.
Report symptoms and changes in condition to the Charge Nurse, and physician.
Maintain accurate, up-to-date, and timely documentation of nursing care provided to all patients.
Ensure problems, actions, specific outcomes, and responses to care are clearly stated and readily available for review.
Rotate weekend/holiday duty as assigned.
Participate in QA/PI activities to ensure continuous quality assurance and process improvement.
Assist in providing services that foster the organization’s mission, values, and goals.
Responsible for timely completion of paperwork required to meet State and Federal regulations.
Documentation should be accurate and detailed to meet these regulations.
Understand and adhere to organizational compliance and privacy programs.
Assists with the orientation of new staff.
Performs other job-related duties as required.
