About The Position

This role supports the Transplant Program by coordinating and scheduling patients for transplant evaluations. The LVN will work closely with the transplant team to ensure patients move efficiently through the evaluation process. This position is ideal for an LVN looking to use their clinical knowledge in a non-bedside, non-patient-facing role with a Monday–Friday schedule, stepping away from traditional bedside care while remaining an important part of the patient journey. The primary responsibilities include clinical review of new referrals, clinical screening with patients, and coordination of care with physician team members. The LVN will provide direct patient care under the direction of a Registered Nurse, utilizing clinical and cognitive skills, specialized and expanded skills, exhibiting broad nursing knowledge and the ability to manage and communicate effectively with the healthcare team.

Requirements

  • Graduate from an accredited school of vocational nursing or licensed practical nursing
  • Current State of Texas license or temporary/compact license to practice as a Licensed Vocational Nurse (LVN)
  • Certified in Basic Life Support
  • Minimum of (3) three years prior LVN experience preferably in acute care setting
  • Organized
  • Detail-oriented
  • Comfortable managing multiple priorities

Responsibilities

  • Coordinate and schedule patients for transplant evaluations.
  • Help facilitate the logistical steps of the transplant evaluation process.
  • Collaborate with nurses, providers, transplant coordinators, and other members of the multidisciplinary team.
  • Maintain accurate scheduling and documentation to keep the evaluation process moving efficiently.
  • Clinical review of new referrals.
  • Clinical screening with patients.
  • Coordination of care with physician team members.
  • Delivers safe and appropriate care to patients, according to the nursing skills inventory and the age and needs of patient/population served, within the scope of licensure.
  • Collaborates with the interdisciplinary team in the delivery of patient care; supporting and collaborating with the Registered Nurse in providing direction to the healthcare team.
  • Participates in a systemic, interdisciplinary, and ongoing evaluation of programs, process improvement and desired client-centered outcomes.
  • Considers the diverse needs of each patient through the entire continuum of care, providing and ensuring the highest level of service according to established service standards, optimizing the patient experience.
  • Adheres to all regulatory and Texas Board of Nursing standards.
  • Performs other duties as assigned, within the scope of practice and clinical competency.
  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
