As a the LVN Weekend Supervisor, you will serves as the charge nurse and weekend supervisor for The Mission, providing leadership and oversight of weekend operations under the direction of the supervisor. Assists with planning, organizing, developing, and directing daily functions and serves as the building’s designated person in charge, addressing staff, resident, and family concerns as they arise. In addition to supervisory responsibilities, provides licensed nursing services within assigned households, participates in resident needs assessments, and supports the development and coordination of individualized care plans. Performs required nursing duties and procedures within the scope of licensure, collaborates effectively with residents, family members, physicians, and other internal and external professionals, and provides direct resident care on the floor as needed.

Requirements

  • High school diploma or equivalent.
  • At least one year of related experience in a long-term care (LTC) facility.
  • Must be an LVN in the State of Texas and in good standing with the Board of Nurse Examiners
  • Possess current CPR, and AED certifications, as well as be current in attaining the Continuing Education Units (CEUs) necessary to meet requirements for licensure.
  • At least one year of previous experience in a long term care setting is highly desirable.

Nice To Haves

  • Certification in Long Term Care is desirable.
  • Knowledge of current nursing practices related to geriatric and dementia care, as well as hands-on experience with same.
  • Supervisory experience in a health care setting is preferred.

Responsibilities

  • Supervises and provides nursing care in accordance with physician orders and in conformance with recognized nursing techniques and procedures, established standards, and administrative policies.
  • Performs duties related to pre-admission, admission, and discharge of residents.
  • Ensures accurate admission information is received and translated to care staff.
  • Supports all admissions and discharges and ensures completeness and accuracy of admission/discharge materials.
  • Responsible for medication regime including acceptance of physician’s orders, transcribing orders, ordering medications, and ensuring medications are on hand.
  • Administers medications as well as supervises medication aides to ensure accuracy in their duties.
  • Ensures medical supplies are accessible; relates need for additional supplies in a timely manner.
  • Maintains a safe environment for residents.
  • Monitors and supervises Health Support Assistants (HSA), Certified Nursing Aide(CNA), and Hospitality Aides ensuring they are providing appropriate care for residents and meeting their needs.
  • Provides HSA with guidance and education as needed to ensure residents’ needs are met and cognitive capabilities and physical functioning is understood by the care givers present.
  • Communicates with family members, providing them with information relating to the residents’ care and condition.
  • Communicates in a professional manner with physicians, other staff, support services, and outside agencies.
  • Acts as residents’ advocate.
  • Records all pertinent information to include observations, treatments, and medications on resident Electronic Medical Record (EMR) and keeps Supervisor informed of any adverse changes in resident condition.
  • Makes periodic rounds of each resident, notes needs and conditions, and reports adverse situations to Supervisor and attending physician.
  • Receives and follows Physician’s Orders and enters them into resident EMRs as needed; notifies other physicians as needed.
  • Provides the oncoming charge nurse a resident report, as required.
  • Follows emergency procedures to include notification, evacuation, treatment, medical emergency, death, fire, and resident transfer.
  • Attends continuing education courses, in-services and training sessions as required.
  • Provides support to Mission weekend staff.
  • Manages staff, resident or family member’s issues as they arise
  • Oversight of admissions, transfers and discharges throughout the facility
  • Provides facility staff with guidance and education as needed to ensure residents’ needs are met
  • Responds promptly and properly to all emergencies and disasters, per established Blue Skies policies and procedures.
  • Making rounds to ensure service excellence.
  • Ensure quality care throughout the facility
  • Other duties as assigned by the Supervisor.
