About The Position

The Licensed Vocational Nurse (LVN) Swing Bed Full-Time position plays a critical role in providing comprehensive nursing care to patients transitioning from acute hospital care to a swing bed program. This role ensures continuity of care by delivering skilled nursing services, monitoring patient progress, and collaborating with interdisciplinary teams to develop and implement individualized care plans. The LVN will be responsible for managing focused patient assessments, administering medications, and performing treatments in accordance with established protocols and regulatory standards. This position requires adaptability to work across various patient care settings within the facility, ensuring high-quality care and patient safety at all times. Ultimately, the LVN contributes to improving patient outcomes and facilitating smooth transitions through effective communication and compassionate care delivery.

Requirements

  • Current and valid Licensed Vocational Nurse (LVN) license in the state of employment.
  • High school diploma or equivalent.
  • Basic Life Support (BLS) certification.
  • Minimum of one year of clinical nursing experience, preferably in a hospital setting.
  • Ability to communicate effectively with patients, families, and healthcare team members.
  • Familiarity with electronic health record (EHR) systems.
  • Strong organizational and time management skills.
  • Demonstrated ability to work independently and as part of a multidisciplinary team.
  • The LVN will utilize clinical nursing skills daily to assess patient conditions, administer medications, and perform treatments with precision and care.
  • Effective communication skills are essential for educating patients and collaborating with healthcare team members to ensure cohesive care delivery.
  • Organizational skills help manage multiple patients and documentation requirements efficiently, maintaining compliance with regulatory standards.
  • Familiarity with electronic health records enhances accuracy in documentation and facilitates seamless information sharing across departments.
  • Additionally, critical thinking and problem-solving skills enable the LVN to respond promptly to changes in patient status and contribute to quality improvement efforts.

Nice To Haves

  • Experience working in a swing bed or skilled nursing facility environment.
  • Additional certifications such as IV therapy or wound care.

Responsibilities

  • Provide direct nursing care to swing bed patients, including medication administration, wound care, and monitoring vital signs.
  • Conduct patient focused assessments and document changes in condition promptly and accurately.
  • Collaborate with physicians, registered nurses, therapists, and other healthcare professionals to develop and update individualized care plans.
  • Educate patients and their families regarding care plans, treatment procedures, and health maintenance.
  • Ensure compliance with all regulatory requirements, facility policies, and safety standards while maintaining patient confidentiality.
  • Assist in coordinating patient admissions, transfers, and discharges within the swing bed program.
  • Participate in quality improvement initiatives and staff meetings to enhance patient care services.
