About The Position

We are looking for an LVN Runner/On-call to join the Assisted Home Health & Hospice Team. This role involves providing comfort, compassion, and care to patients and families in their end-of-life journey. The position requires making visits to patients during on-call shifts within the service area, ensuring quality and safe delivery of hospice care nursing services, and communicating with the RN Case Manager and physician regarding patient condition and needs. The LVN will assist the RN in carrying out the plan of care and perform procedures according to established standards and agency policies.

Requirements

  • Graduate of accredited nursing program.
  • Current, active, license as an RN or LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
  • Current CPR, health clearance and clinical skills self-assessment.
  • Successful passage (80%) of Agency competency exam and core competency skills checklist.
  • Minimum of one year experience as an RN or LVN.
  • Knowledge of Hospice care and services.
  • Hospice experience is required.

Responsibilities

  • Deliver holistic, end-of-life care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation.
  • Effectively interact with patients, caregivers, and other health team members while maintaining standards of professional nursing practice.
  • Make visits to patients during on-call shift within the service area.
  • Ensure quality and safe delivery of hospice care nursing services.
  • Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient's condition and/or needs.
  • Assist the RN in carrying out the plan of care.
  • Perform procedures at a level of care standard within the medical community, scope of practice consistent with the Vocational Nursing Practice Act and follows Agency's current policies and procedures.
  • Document clinical and progress notes that show provision of and outcomes of interventions.
  • Provide service within the Agency's catchment area.
